Absolute Contraindications
These conditions make HBOT genuinely dangerous and require avoiding therapy entirely:
- Untreated pneumothorax (collapsed lung): Pressure changes could rupture the lung. Once surgically treated, HBOT may be safe.
- Certain chemotherapy drugs: Bleomycin (lung toxicity), doxorubicin (cardiac toxicity), cisplatin (potentially). Wait period required after last dose.
- Disulfiram (Antabuse): Inhibits oxygen toxicity protection.
- Cis-platinum (during treatment)
- Untreated severe claustrophobia: Not strictly medical, but practically unworkable.
Relative Contraindications
These conditions require careful evaluation โ HBOT may be possible with adjustments:
- Severe COPD with air trapping: Risk of barotrauma during depressurization
- Asthma (uncontrolled): Should be well-controlled before HBOT
- Active upper respiratory infection: Wait until resolved
- Active ear infection or recent ear surgery: Wait for clearance
- Untreated diabetes (severe hyperglycemia or hypoglycemia)
- Pregnancy: Generally avoided except for specific severe conditions
- Seizure disorder: Increased seizure risk at higher pressures
- Implanted devices: Pacemakers and other devices must be HBOT-rated
- Severe congestive heart failure
- Recent eye surgery
What to Disclose
Always disclose to your HBOT team:
- All current medications (prescription and OTC)
- All medical conditions, past and present
- Recent surgeries (especially eye, ear, lung)
- Cardiac conditions
- Any implanted devices
- Pregnancy status
- Smoking history
- Diabetes status
- Cancer history (especially with chemotherapy)
- Mental health conditions (especially severe claustrophobia)
Common Side Effects (Not Contraindications)
These are normal experiences, not reasons to avoid HBOT:
- Ear pressure: Most common, easily managed with equalization techniques
- Sinus pressure: Similar to ears
- Temporary myopia (vision change): Especially with long protocols, reverses after treatment
- Fatigue after sessions: Especially early in protocol
- Hunger or thirst
- Mild headache
Stop Therapy If You Experience
During an HBOT session, alert your technician immediately if you experience:
- Severe ear pain that won't resolve
- Tunnel vision or visual disturbances
- Chest pain or shortness of breath
- Severe anxiety or panic
- Twitching or seizure activity
- Sudden ringing in ears
- Numbness or unusual sensations
The technician can pause and slowly depressurize the chamber if needed. Don't try to be brave โ communicate any symptoms.

- Have medical supervision during sessions
